This is a Stage Monitor with Vocal Effects from TC Helicon, which was introduced in 2014. With its 3-channel built-in digital mixer, the compact TC-Helicon VoiceSolo FX150 can act as your monitor, instrument amp, or all-in-one PA system for smaller gigs. Factory Reset:

To restoring the factory settings of TC Helicon VoiceSolo FX150, first power on while simultaneously holding the all three LOW buttons (in the EQ section) until the unit has fully powered up. The Factory Reset procedure is complete.
